The Office of Administrative Services - Pupil Services is committed to supporting the educational mission of Berkeley County School District. The office is responsible for managing student attendance appeals, expulsions, administration and implementation of school attendance lines and district athletics.

EXTRACURRICULAR PARENT INFORMATION
The value of extracurricular activities upon students’ well-being has been validated through many research studies and Berkeley County School District stands strong in our advocacy for these programs.

PREPARTICIPATION PHYSICAL EVALUATION FORM (PDF)
This form is to be filled out by the patient and parent prior to seeing the physician. The physician should keep a copy of this form in the chart.

EXTRACURRICULAR ACTIVITIES APPLICATION (PDF)This application is to be filled out by the designated individuals and the parent/legal guardian must present this application with all of the requested documents and applicable fees to the Office of Administrative Services – Pupil Services located at 107 East Main Street, Moncks Corner, SC 29461. This application is required for each activity in which the Charter or Home School student seeks to participate.
